Day 49 July 27 2020 Accentuate The Negative
Ive been such a gung-ho, happy camper during my treatment journey, that I think its time to accentuate the negative today. Or at least to try, for the sake of balance.
The trigger for this was a phone call I received the other day informing me that a good friend of mine is in the hospital with sepsis. She takes for her Crohns disease a popular biologic that is used for both psoriasis and Crohns. While there is no way to know what led to her sepsis, one theory advanced by the hospital physician is that her immune system may have been compromised by the treatment, making it easier for sepsis to take hold.
This may not be the case after all, many, many people who have never taken such treatments also have developed sepsis. Still, its a good reminder that serious side effects are possible. I read once its wise to ask your doctor, before starting a psoriasis treatment, the likelihood of a serious side effect occurring over 10 years, since many people require decades of psoriasis treatment.
My friend is awake and sounds pretty good Ill keep you posted on her condition.
Ive also had an itchy scalp again recently. Maybe its because I am using a shampoo that is an incredible $1 a bottle! But maybe its because my scalp misses the T-Gel I had been using for roughly 25 years until I began this new treatment. I am going to switch back again to the T-Gel for a while and maybe even splurge for a $5 bottle of regular shampoo in the near future.

Psoriasis And Types Of Psoriasis
Knowing which sort of psoriasis you have encourages you and your primary care physician to make a treatment arrangement.
- Plaque Psoriasis: This is the most well-known type with raised, excited, red skin covered with shimmering, white scales which may tingle and burn.
- Guttate Psoriasis: This sort frequently begins in kids or youthful grown-ups. It causes little, pink-red spots on your skin.
- Inverse Psoriasis: it causes patches of skin that are brilliant red, smooth, and gleaming, yet dont have scales. It starts to deteriorate with perspiring and scouring.
- Pustular Psoriasis: This sort of psoriasis is exceptional and for the most part shows up in grown-ups. It causes discharge-filled knocks encompassed by red skin.
- Erythrodermic Psoriasis: This sort is the most un-normal, however, its intense. It influences a large portion of your body and causes far and wide, blazing skin that has all the earmarks of being scorched.
- Nail Psoriasis: Nail psoriasis is significantly more normal in individuals who have psoriatic joint pain, which influences your joints.
Wet Dressings And Warm Baths With Salts Or Oats
Baths and showers can be relaxing, but those that are too long or too hot can strip the skin of its oils, and this can make psoriasis worse.
Some people find that a warm bath containing colloidal oatmeal or Epsom salts is soothing and relieves symptoms.
According to , an oatmeal bath or a wet dressing can reduce itching, and a warm bath containing a suitable bath oil can help moisturize the skin.
In 2005, researchers found evidence that Dead Sea salts might help with dry skin. Volunteers immersed a forearm in water with a 5-percent concentration of magnesium salts, the most common minerals in the Dead Sea, for 15 minutes.
The participants skin barrier function improved, their skin hydration was better, and they had reduced roughness and inflammation compared with the control group who used tap water instead.
After bathing, applying an appropriate moisturizer while the skin is still damp can help prevent moisture loss.

Day 56 August 3 2020 Clear
The original plan was that I would chronicle the first 12 or 16 weeks of my new treatment.
But lets face it. We are eight weeks in and: I am clear.
So . . . last one to go, remember to turn out the lights and lock the door!
I will check in periodically in the weeks ahead to the extent I can think of something original to say. Or if Annie Leibowitz does a photo shoot of me. And then once a month through the balance of 2020.
It wasnt too long ago that a dermatologist tried to determine what percentage of my body surface was covered in psoriasis. After he got to 50% and was clearly bored, with more body surface still to cover, he just had the assistant write in my chart 50%+ and we moved on.
Now I am at 0%.
Do I wish this treatment had been around when I was 20? 30? 39? H-E-double hockey sticks yeah. But I am still deeply grateful to have this treatment at age 39+.
So heres me in advice mode: if psoriasis is disrupting your life, or you are simply tired of it, go see a dermatologist and discuss your options. You can probably do one of those video conference appointments from your home and not even put shoes on! It might just be a game changer.
I wanted to close today with the most ridiculous photos I could find. I thought about using outtakes if you thought the photos I have posted were poorly-done, you should see the ones I havent posted! Maybe Ill do that in the coming weeks but I chickened out on that for today.

Should You Remove Psoriasis Scales
Removing the scaling caused by scalp psoriasis is safe to do when done with care. Avoid pulling at existing scales. Instead, treat psoriasis scales with active ingredients that soften them and help them break off from the scalp. The safest physical removal of scales is from shampooing and gently combing the scalp.
What If Those Psoriasis Treatments Dont Work
If psoriasis doesnt improve, your healthcare provider may recommend these treatments:
- Light therapy: UV light at specific wavelengths can decrease skin inflammation and help slow skin cell production.
- PUVA: This treatment combines a medication called psoralen with exposure to a special form of UV light.
- Methotrexate: Providers sometimes recommend this medication for severe cases. It may cause liver disease. If you take it, your provider will monitor you with blood tests. You may need periodic liver biopsies to check your liver health.
- Retinoids: These vitamin A-related drugs can cause side effects, including birth defects.
- Cyclosporine: This medicine can help severe psoriasis. But it may cause high blood pressure and kidney damage.
- Immune therapies: Newer immune therapy medications work by blocking the bodys immune system so it cant jumpstart an autoimmune disease such as psoriasis.

Is There A Cure For Psoriasis
“The Healthy Geezer” answers questions about health and aging in his weekly column.
Question: Is there any cure for psoriasis?
There’s no cure for psoriasis yet, but there are many ways to get relief from the symptoms of this troublesome disease.
Psoriasis is one of the most common skin ailments. More than 125 million people throughout the world have psoriasis. This chronic disorder affects people of all ages. Psoriasis seems to be inherited.
The condition causes skin redness and irritation that can appear anywhere on the body. Most people with psoriasis have thick, red skin with flaky, silver-white patches. There is no known way to prevent psoriasis. It is not contagious.
Psoriasis is the most common autoimmune disease. Researchers think it probably occurs when the body’s immune system attacks healthy cells, mistaking them for dangerous substances. Up to one-third of people with psoriasis may also have a form of arthritis called “psoriatic arthritis,” in which the immune system also attacks the joints.
In many cases, psoriasis goes away and then flares up again. The triggers that bring on psoriasis include: stress, dry air, infections, skin injuries, some medicines, too much or too little sun, cold weather, drinking too much alcohol and smoking.
If you have a weakened immune system, you may experience more severe symptoms from psoriasis. A weakened immune system comes with rheumatoid arthritis, chemotherapy and AIDS.
There are a variety of psoriasis types. These include:
Hair Care Can Trigger An Outbreak
Any hair care that causes pain can trigger a new outbreak of psoriasis on the scalp. Maybe the hair is combed a little too vigorously. Perhaps the water for the shampoo is too hot. Or there is an allergic reaction to a dye or a hair care product. When this happens, the nerves in the scalp generate a neurotransmitter called substance P, the pain chemical, and the skin gets a signal to start an incredibly rapid repair process. Skin can produce 10,000 times as many new skin cells as are needed to repair microscopic damage, and redness, flaking, drying, and cracking start all over again.

Progress Toward A Cure
Each new treatment represents research and gained knowledge about what psoriasis and PsA do to your body. That helps get medical science closer to understanding the underlying causes and mechanisms.
Eventually, that understanding could lead to better diagnostics, better treatments, preventive measures, and possibly even a cure.
For now, though, successful treatment and remission are the goals to strive for.

Topical Treatments For Psoriasis
These are drugs you rub directly on your skin. Along with a good moisturizer, theyâre usually the first thing your doctor will suggest, especially for mild to moderate psoriasis. There are over-the-counter and prescription options.
Topical treatments for psoriasis come as ointments, creams, or foam and include:
Steroid creams. These slow down immune cells in your skin. They can ease swelling and redness. Mild steroid creams are available over the counter. Youâll need a prescription from your doctor for something stronger. Steroids come with side effects and shouldnât be used on sensitive areas like your face or genitals. They can burn or thin the skin. Use them exactly the way your doctor tells you.
Salicylic acid. This can soften and thin scaly skin. But it can also irritate your skin if you leave it on too long. It might weaken your hair follicles and cause temporary hair loss, too. The body can absorb salicylic acid if you put it on large patches of skin.
Calcipotriol . This is a strong form of synthetic vitamin D. Itâs known to control overactive skin cells. Your doctor might pair it with a steroid cream.
Tazorac is available gel or cream and applied one and twice daily. it is ot recommended for those who are pregnant or breast-feeding or intending to become pregnant.

More Than Skin Troubles
A 2017 study from the Journal of the American Academy of Dermatology found that people with psoriasis that covers 10% of their body or more are 64% more likely than those without psoriasis to develop type 2 diabetes. “About 30% of people with psoriasis also might develop psoriatic arthritis, which causes destructive inflammation in your joints,” says dermatologist Dr. Gideon Smith. Psoriasis also may signal a higher risk for fatty liver disease and heart attacks.
How Can Psoriasis Be Permanently Cured
Answered by: Prof DM Thappa | Associate Professor and Head, Dermatology and STD Department, JIPMER, Pondicherry
Q: I am a 32 years old male suffering from psoriasis on my knees, elbows, fingers, palms and it affects 20% of my body. I dont take any oral medication. I do apply coal tar and salysilic acid ointment. It softens the skin only. I am getting into depression gradually with the progression of the disease. How can psoriasis be permanently cured? I dont want it to spread it to other parts of my body. I do yoga regularly.
A:You need to be considered for either phototherapy or systemic therapy or combination of both. Only topical therapy for psoriasis affecting more than 20% surface area may not be suffice. Your psoriasis surely can be controlled. Winter season do aggravate this.

Day 14 June 22 2020 Holy Moly Batman
I am going to write in a subdued fashion today because someone joked that perhaps I was over-caffeinated in my previous post. The point is well-taken: I dont want my words to obscure what is happening to my skin.
It is Day 14. After decades with psoriasis on more than half of my body, I am two whole weeks into my new treatment.
So I urge you to click on these thumbnail photos to get the ridiculously large versions. Fourteen days. It seems almost impossible. Im desperately resisting the urge to go ALL CAPS here. So again, Ill lay off the ALL CAPS if you will click on these three photos.
What do you think? Yall have not crashed the internet yet sending me comments and questions, so I invite you once again to do so.
Since no one has invented touch-a-vision yet and yet my kid complains that everythings already been invented I will try to describe how my skin now feels, even though description is not my strong suit. I simply cannot believe how soft the skin is on my stomach and back, on my arms and my legs. I guess after 30 years, you can actually forget the feeling.
Now I could shake hands during a business meeting were those still things in the age of COVID without the other person instinctively glancing at my hand to wonder why they are clasping sandpaper.
I could go to anyone to cut my hair right now if I could get an appointment as they re-open for business and my neck and behind my ears and my scalp would not draw their attention or concern.
How Is Psoriasis Diagnosed
Doctors usually diagnose psoriasis by examining the skin, scalp, and nails. They’ll also ask whether someone else in your family has psoriasis and if you recently had an illness or started taking a new medicine.
Rarely, doctors might take a skin sample to check more closely. A can tell the doctor whether it’s psoriasis or another condition with similar symptoms.
